Another Gift Wrap Question...

I have seen several posts regarding the use of radio buttons and text fields to record additional information, but how would one use a checkbox to indicate an item should be gift wrapped - I suppose I could use yes/no radio buttons, but a simple checkbox seems cleaner to me...

Alternatively, if I were to use a drop down menu with "No" as the default and "Yes, $1.00" as the only alternative, how would I add this cost into the total cost in the managecart and checkout pages?
